Inc.) is a nonprofit, nonsectarian corporation committed to providing

and operates two highly regarded retirement communities in Connecticut,
Avery Heights in Hartford and Noble Horizons in Salisbury.

complete continuum of care, including adult day care, home health care,
assisted living, subacute care, short- and long-term rehabilitation,
and outpatient rehabilitation as well as intermediate and skilled
nursing care. This continuum allows CHI to respond to the unique health
care needs of each and every one of our residents. For example, if an
apartment dweller at Avery Heights or a cottager at Noble Horizons
undergoes heart surgery, the subacute unit at Avery or Noble can
oversee nursing care and rehabilitation following discharge until the
resident can return to independent living or perhaps to another level
of care.
