Church Homes Inc., is a not-for-profit, nonsectarian corporation, which owns or manages two Connecticut retirement communities: Avery Heights, in Hartford and Noble Horizons, in Salisbury.

Welcome to Avery Heights Village Church Homes and each of its villages is dedicated to a mission of supporting older adults at the highest level of independence possible. Our villages provide a wide range of lifestyle options beginning with cottages and apartments for independent living. Health care services include assisted living, subacute care, rehabilitation, intermediate and skilled nursing care.

Many supportive services play a vital role in the continuum. The villages of Church Homes provide recreational programs, certified pastoral care, and social work services to our residents.

The continuum of care also provides a unique opportunity to live in an environment that responds to the health care needs of each resident. For example, if an apartment dweller at Avery Heights undergoes heart surgery at a neighboring hospital, the subacute unit at Avery can oversee nursing care and rehabilitation following discharge from the hospital until the patient is well enough to return to independent living or perhaps to another level of care.

Outpatient services are also available. At Avery Heights the Home Health Agency brings nursing and other supportive services to the homes of residents and older adults in the community at large. The Rehabilitation Center at Avery offers physical, occupational and speech therapies on an inpatient and outpatient basis. The Center’s goal is to restore the highest level of independence possible following illness, accident, or surgery. Cottages and Apartments
